Confidential HR Request Form

  • Used to reach out directly to the PAST Associate Director of HR to discuss any confidential topic.

Expansion of Duties Form

  • Expansion of duties for OAs who are taking on more responsibility and/or complexity in their role. This can be either with or without a pay increase. 
  • Used for Division of Global Engagement move to Sr. GEPC or Sr. ISSA

New Recruitment or Direct Appointment Form

  • Hire for a current or upcoming vacant position
  • Create and hire a new position
  • Direct appointments for special circumstances

Onboarding Questions

  • Ask questions about onboarding process or specific employees. 
  • Note: New employees hired through PAST will automatically have an onboarding ticket created so managers can track the process.

Performance Evaluation Submission Form

  • Attach classified, OA, or Faculty annual performance evaluations.

Reclassification Form

  • Used when a classified employee is moved from one classification to another classification, typically when taking on increased or new responsibilities.

Revised Position Description Form

  • Used when adjustments to already approved PD's are made. (Duties, requirements, tasks, etc)
  • Note: for position descriptions that are updated at the same time as a performance evaluation, attach the PD to the Performance Evaluation Submission form

Separation/Transfer Form

  • Leaving position at UO
  • Leaving position and moving to position in another department or same department
  • Leaving position but still employed by UO in another position
  • Students
  • Retirement

Stipend and Cash Awards Form

  • One-time activities outside scope of current duties
  • Temporary additional activities outside the scope of current duties
  • Coverage of vacancy 

Student Hire Form

  • Used to process student hires after student has been selected for the position

Supervisor Change Form

  • Used to remove or update a supervisor for new employees, leaving employees, or reorganizations within the department. 

Temporary Hire Request Form

  • Temporary work within the department to cover a short term need. For example, during the search for a permanent employee.
  • Note: PAST can help identify qualified candidates through the temp pool if a department does not have a candidate in mind

Title Change Form

  • Used for updating employee title without changes to salary or job duties
